Palliative care: We offer support in the life journey following the decision for palliative care to begin, including starting care as soon as the diagnosis has been received. Essentially, we offer assistance to our service users, to get the most out of their life, and enjoy their independence for as long as possible.

End of life care (Including NHS CHC fast tracked end of life care): With the first hand experience and appreciation of the time factor in CHC fast track processes and end of life care, we work to complete an assessment and commence a package of care within six hours from receipt of the referral.

We recognise our role in ensuring that people are able to die in dignity in their place of choice, in those precious last hours. 

We also understand the potential benefits to the families, for working in partnership with them and taking their own needs into consideration.
